Personal Health Record & Connected Sources

A Personal Health Record (PHR) is patient-controlled. It can include manually entered information, uploaded documents and data synchronized from approved external systems.

Recommended ForMyMedical structure

My Health Record — patient-owned overview.
Labs & Results — blood work and other results.
Documents — reports, discharge papers, referrals and patient-selected files.
Measurements — patient-selected trends such as weight, blood pressure or glucose.
Connected Sources — provider, lab and device synchronization.

Technical terminology

FHIR means Fast Healthcare Interoperability Resources. It is a standard used by many modern health systems to exchange structured health information through APIs. Availability and data access vary by organization and jurisdiction.

Production boundary

Before ForMyMedical stores or synchronizes real medical information, it needs authenticated users, encryption, consent/sharing controls, audit logs, secure hosting, backup/recovery, retention/deletion rules and privacy/security/legal review.
