ForMyMedical Privacy Notice

ForMyMedical is a health-organization and communication platform. This notice explains how information submitted through the public intake form is handled.

What we collect

Name, email, optional phone and location, inquiry type, preferred contact method, and the message you choose to submit. We also retain a one-way hashed network identifier and basic browser information for abuse prevention.

How sensitive fields are stored

The message body and phone number are encrypted at the application layer before database storage. Office notification emails intentionally exclude those sensitive fields.

Why we use it

To respond to your request, organize follow-up, protect the service from abuse, and maintain an audit trail of intake handling.

Your choices

Submit only what is necessary. Do not use ForMyMedical for emergencies. You may contact office@4br.ca to request access, correction, or deletion where applicable.

ForMyMedical does not claim compliance with a specific regulated-health-data framework until the applicable legal, security, hosting, retention, and operational requirements have been formally reviewed for the market in which the service is used.
